Sawm and Tahaarah of a woman who inserts her finger or another object in her vagina

Question: If a woman inserts her finger into her vagina while making Istinjaa’ (cleansing the private parts with water after urination or defecation), applying ointment, or inserting a suppository; or if a female doctor inserts a diagnostic instrument or her finger during a medical examination, should a woman perform Ghusl (full ritual bath)? If this occurs during the daytime of Ramadaan, should she break her Sawm (Fast) and make up for it later?
